Medication Aide

The Health Certification Project offers two written certification exams for nurse aides who administer medication:

• Certified Medication Aide
• CMA-Insulin Administration.

Testing Prerequisites

• Candidate must currently be certified in Oklahoma as a long-term care, home health care, or developmentally disabled direct care aide
• Candidate must complete an approved training program

Certified Medication Aide candidates must also attest that they are at least 18 years of age, received a high school or general equivalency diploma, hold a current Oklahoma nurse aide certification with no abuse notations, have six months experience as a CNA and are physically and mentally capable to safely perform duties.

Required Documentation

• Original Training Verification Form signed by the training program supervisor or approval letter from NAR
  
• Valid, current photo identification issued by government entity within the U.S. (i.e. passport, driver's license, school-issued ID)
• Proof of the SSN:­
  • Social Security Card
  • Letter from Social Security Administration
  • W-2
  • Official Tax Return

  In order for the advanced designation to be noted on the Oklahoma Nurse Aide Registry, the candidate must send the completed ODH Form 504, CMA Diabetes Care Application , and the required fee to the Oklahoma State Department of Health.
